  • FormScore is a solution that helps managers understand colleague wellbeing, effectiveness, and commitment to the team. It is a real-time measure of performance readiness and the factors that are driving it.

    At the same time, FormScore gives employees a valuable resource to help manage their wellbeing and performance.

    1. Start your team meetings, or all-hands calls, with a 2-minute FormScore check-in and give your employees the opportunity to show how they feel, anonymously.

    2. Customise the check-in based on the factors that affect your people the most and get real-time insights.

    3. Understand the impact of hybrid working on team performance and wellbeing.

    4. Overlay with company and world events to understand the impact of pay reviews, restructures, CEO announcements, and more… and view in the manager dashboard.

    5. Not only that. As a manager, you get instant and personalised suggestions to address areas that employees are struggling with.

    6. Built with managers in mind, means you don't have to download additional software on your work laptop or have to jump through IT & Security hoops. Available as a web solution, use as often as needed with minimum effort.

    1. Navigate to https://myform.today/ and create your first FormScore Poll using your work email address.

    2. Record your own FormScore - scan the QR code (or copy the URL underneath the QR into a new browser) and complete the poll.

    3. Ask your team members to scan the QR code (or share the score link) on the top right to record their FormScore. It can be helpful if you talk through your own FormScore and the drivers you selected.

    4. Reassure your team that their individual data is not shared with you or the HR team

    5. Use the 'Switch View' feature and the 'Drivers' tab to share the 'Big Picture' with your team. This promotes openness and cultivates psychologically safe workplaces.

    • For managers: An account is created automatically for you based on the work email you provide when you first sign-up.

    • For employees: no account is required to use the check-in. They can optionally create an account to access their score history and trend data.

    • We process a minimal amount of personal data for managers (email address, team name)

    • This is by default a "no personal data" web-solution. We do not share any of your individual data, including your FormScores, with your employer. Full Stop. Period.

    • Your team's average score is based on aggregate FormScores and individual scores cannot be traced by anyone.

  • You can make use of Check-in solution as part of the two-week free trial. This allows you to utilise the check-in for a single team. To unlock the Check-in tool for all managers and teams in your organisation, please proceed with the Org or Enterprise plan.
