How to delete or reactivate my account

To delete your account:

1. Navigate to the Me tab

2. Settings icon (top right)

3. Delete Account > a pop-up appears for you to confirm that you wish to delete your account

To reactivate your account:

If you deleted your account but would like to continue to use FormScore:

1. Please get in touch with us on LiveChat or by emailing us on [email protected].

2. You will be asked to provide the phone number associated with the account so that we can reactivated it.
