Log a score

1. Navigate to the FormScore app/plug-in within Microsoft Teams

2. In the chat/BOT window, type Help

3. Hit the Send button to see all options available

4. Type Set Score, hit Send to log your score

5. From the drop-down list, click on the score that represent your Form

   • 10 being a lifetime high, 6 being moderate form and 1 the lowest form for you

6. Then choose your Drivers of Form

   • Pro tip: you can select more than one drivers from each category
