How to complete a poll
To begin, your manager will share their screen and guide the team through the first poll.
Please note, that all scores are anonymous and individuals are in no way identifiable.
To record your FormScore, use your phone's camera to scan the QR code, shown at the top right of your manager’s screen.
Your manager might share the poll URL in the meeting chat, giving you the option to complete the poll on your desktop.
Adjust the sliders to the score that represents your Wellbeing, Effectiveness, and Commitment to the team. (1 being the lowest score, 10 being the highest score).
To find out what ‘Wellbeing, Effectiveness, and Commitment’ is about, use the ℹ️ icon next to each category.
Select your current work location: Remote/In Person.
Select the work and personal drivers affecting your form. (You can select as many as you like.)
In the first tab, choose the drivers that boost your score. These will appear in blue.
Then navigate to the second tab to choose the drivers that lower your score. These will appear in orange.
