What happens when I log a low score

FormScore for Humans

  • When you log a low score of 4 or lower, a notification will be sent to those you are friends within the FormScore app, to make them aware them that you are feeling low. (See notification on the right.)

  • If you log a score of 2 or 1, a video will pop-up with advice on what to do, how to ask for support and how to keep yourself safe. You can also watch the videos on what to do when you experience each score by navigating to the Today tab > (i) icon top right > click on any of the 10 colourful tiles to watch a video for that particular score.


FormScore for Teams

If you use FormScore in your organisation and you are a member of that team:

  • When you log a low score of 4 or lower, a notification will be sent to those you are friends within the FormScore app, to make them aware them that you are feeling low.

  • If you log a score of 2 or 1 you have access to resources provided by Form and your organisation. In addition, the wellbeing admin in your organisation will be notified that someone logged a very low score. This notification is always anonymous but it will help the wellbeing admin reach out to that team.

Resources provided by your organisation:

Note that these vary based on your organisation’s set up and your location, therefore you may only see some of the options listed below.

  • If you are in the UK, you may see the option to contact the Samaritans or SHOUT, with support available 24/7.

  • If your organisation offers an Employee Assistance Programme you can navigate to that page to ask for support.

  • Depending on your organisations set up, you may have the option ‘Share my details’. Remember that by default, we do not share any of your personal data, including your FormScores, with your employer. This is the only situation where you have the option to choose to share with your employer that you currently experience low form. Your historic scores will not be shared with your representative.

If you Agree, the following email will be sent to your organisation’s representative. This is usually someone in the HR or Wellbeing Team who has previously been in touch with employees regarding the use of FormScore in the organisation.

  • Based on your organisations set up, you may see the option ‘I want to be contacted’. This works similarly to the ‘Share my details’ option, in that your organisation’s representative is made aware that you currently experience low form. The added benefit of using this option, is that you can write a quick note to your organisation’s representative in an email, if you wish to request specific help.
